MEMORANDUM FOR MADE A MISTAKE, AMN,
FR123-45-6789
FROM: 4 XXX/CC
SUBJECT: Letter of Reprimand - Unauthorized use of Government Travel Card
1. Investigation has disclosed that
between 6 Apr 03 and 7 Apr 03, you made unauthorized
purchases with your Government Travel Card. Specifically, you used your
Government Travel
Card 12 times while you were not in an authorized travel status.
2. You are hereby reprimanded. You have engaged in aberrant behavior which
will not be
tolerated from a member of the United States Air Force. Your past behavior has
raised serious
questions about your ability to remain in the United States Air Force. You have
acknowledged
and signed a lawful order not to use the Government Travel Card except for
authorized official
travel expenses. These include food, lodging, and transportation expenses. By
using this card
for unofficial purposes, you have violated that order. I want you to know that I
consider
disobeying a lawful order to be one of the most serious offenses an Air Force
member can
commit. Strict adherence to orders is the only way that any military force
anywhere in the world
can ever execute its mission, whether that mission is offensive or defensive.
The ###th
Communication Flight is no different; you have damaged our mission capability by
identifying
yourself as one who cannot be depended upon to perform as ordered. You are going
to have to
strive diligently if you ever hope to reestablish your dependability. A
repetition of this totally
unacceptable behavior will result in stronger action against you.
3. PRIVACY ACT. AUTHORITY: 10 U.S.C. 8013. PURPOSE:
To obtain any comments
or documents you desire to submit (on a voluntary basis) for consideration

4. You will acknowledge receipt and return this letter to me within 3
workdays of your receipt.
Your signature on this document is solely for receipt purposes and is not an
admission of guilt.
Any comments or documents you wish to be considered concerning this LOR must be
