SUBJECT: Letter of Reprimand - Failure to Report
1. Investigation has shown that you
did, on or about 18 December 1999, fail to go at the time
prescribed to your appointed place of duty, to wit: Building 334, 36th Tactical
Fighter Squadron,
Osan Air Base, Republic of Korea, in violation of Article 87 of the Uniform Code
of Military
Justice (UCMJ). This offense is so serious, that had I elected to punish you
under the provisions
of Article 15 of the UCMJ, you could have received a reduction in rank,
forfeiture of 1/2 of your
base pay for two months, and 60 days restriction with 45 days extra duty.
2. You are hereby reprimanded. You have engaged in aberrant behavior which
will not be
tolerated from a member of the United States Air Force. Your past behavior has
raised serious
questions about your ability to remain in the United States Air Force. Your
actions have brought
discredit upon yourself and require me to seriously question your judgment and
sense of
responsibility. This is not the first time you have failed to arrive to work on
time. Your
supervisor has counseled you on several occasions concerning your apparent
disregard for the
normal duty hours.
3. PRIVACY ACT. AUTHORITY: 10 U.S.C. 8013. PURPOSE:
To obtain any comments
or documents you desire to submit (on a voluntary basis) for consideration
concerning this
action. ROUTINE USES: Provides you an opportunity to submit comments or
documents for
consideration. If provided, the comments and documents you submit become a part
of the action.
DISCLOSURE: Your written acknowledgment of receipt and signature is
mandatory. Any
other comment or document you provide is voluntary.
4. You will acknowledge receipt and return this letter to me within 3
workdays of your receipt.
Your signature on this document is solely for receipt purposes and is not an
admission of guilt.
Any comments or documents you wish to be considered concerning this LOR must be
included
with your response.
