MEMORANDUM FOR MADE A MISTAKE, AMN,
FR123-45-6789
FROM: 4 XXX/CC
SUBJECT: Letter of Reprimand
1. On Thursday, 30 November 2006, ??? late travel
vouchers were discovered on your desk. These vouchers did not have the recovered
cover letter detailing the reason the vouchers were being held.
2. On Thursday, 30 November 2006, you were formally counseled by TSgt <LNAME>
for this same duty failure. You have also been admonished by MSgt <LNAME> and
MSgt <LNAME2> for late vouchers discovered 26 August 2006 and 5 Sep 2006.
3. Your failure to do your job caused a total of $XXX of late payments. This
amount is $XXX Government Travel Card payments that were made late. Also, $XXX
in member payments were not paid in a timely manner. Due to your negligence the
entire section may not meet the Financial Management Indicator for December
2006.
4. This action is inexcusable and yet it continues. Again, your failure to do
your job as required causes late vouchers against the required ACC & AFAFO
mandate of 95% paid in five days. Not only does your disregard for timely
payments hurt your Travel Team, is also means our customers do not receive their
payments in a timely manner, and possibly Bank of America for GTC payments. Your
actions reflect poorly on the Travel Team as a whole for the <BASE> AFB
community. Not only does it reflect poorly on Travel Pay, but the squadron as
well. Attached is a detailed list of the late vouchers.
5. You are hereby reprimanded for violating Article 92, Failure to obey order or
regulation. I fully expect for this not to happen again. Not only does it
reflect poorly on your ability to perform your job, it also reflects poorly on
the Travel Pay section and the ## Comptroller Squadron as a whole.
6. PRIVACY ACT STATEMENT. AUTHORITY: 10 U.S.C.8013. PURPOSE: To obtain any
