SUBJECT: Poor Physical Fitness of RANK FNAME MI. LNAME
1. I received an “Individual Fitness Assessment History/Individual Test History”
on RANK LNAME providing evidence that RANK LNAME failed Fit tests on DATE1,
DATE2, DATE3, and DATE4. AFI 36-2905, Fitness Program, paragraph 9.1.5.2.,
requires that I recommend discharge or retention if a member of my squadron
receives four poor scores in less than 24 months, as is the case with RANK LNAME.
2. ADDITIONAL INFORMATION AS NEEDED
3. RANK LNAME is fully aware of the consequences of not passing his PT test. At
this time, I recommend discharge. He is currently scheduled for a medical
physical on DATE.
